14)  Migration Certificate, Admit Card of Board & Council in case of outside Odisha candidates

15)  Physical Fitness Certificate / Medical Fitness Certificate.

16)  No Objection Certificate from the employer (only for P.B.B.Sc. & M.Sc. Nursing Course)

17)  GNM Pass Certificate, Mark Sheet &  Valid Registration Certificate (only for P.B.S.Sc. Nursing Course)

18)  Certificate & Mark Sheet of GNM & P.B.B.Sc. (N) / B.Sc.(N) and valid Registration Certificate of P.B.B.Sc. (N) / B.Sc.(N) (only for M.Sc. Nursing Course)

19)  One Year Clinical / Teaching Experience Certificate from the date of registration to last date of application after completion of P.B.B.Sc. (N) / B.Sc. (N) course (only for M.Sc. Nursing Course)
